A Little More About What You Will be Doing

The Supportive Care Medicine (SCM) Triage Nurse provides initial triage assessment of inpatient referrals to the Supportive Care Medicine consultative service and helps to assess the urgency of new consults. This position will work with other members of the interdisciplinary team to help manage the department’s overall consultation workflow and be a primary resource/contact for referring clinicians and the patient’s clinical team. They will work with the Supportive Care Medicine team and the patient’s primary team to ensure that patients experience seamless transitions from inpatient to outpatient environments, assist with initial as well as follow-up visits for patients who are on-service, and function as an integral part of the interdisciplinary team. The Supportive Care triage nurse must possess strong communication skills, clinical acumen, critical thinking, thoughtful decision-making and organizing abilities, as well as interpersonal skills to facilitate teamwork and to support the SCM team and clinical staff.

Job duties and responsibilities:

  • Conducts initial and follow-up triage nursing assessments for SCM
  • Identifies priorities for patient care management
  • Enacts problem-solving and critical thinking skills
  • Reports concerns or urgent messages to the interdisciplinary team
  • Collaborates in providing patient care
  • Develops and maintains therapeutic relationships
  • Promotes patient safety
  • Facilitates smooth transitions between care environments
  • Provides effective patient/family/staff teaching
  • Provides educational in-services to peers and referral sources on the topic of Supportive Care Medicine
  • Utilizes resources effectively and efficiently
  • Implement transitions of care between inpatient Supportive Care Medicine and continued outpatient follow up
  • Adheres to scope of practice/Cedars-Sinai standards and facilitates continuous improved processes/services
  • Maintains and enhances professional skills

Qualifications

Education:

  • Graduation from an accredited school of nursing required
  • Bachelor of Science in Nursing degree preferred

License/Certifications:

  • Valid CA RN license required
  • Certified Case Management (CCM) RN preferred

Experience: 

  • 3 years of acute nursing experience required
  • Experience in palliative care, oncology or critical care and/or project management preferred.
