The Senior Manager, Post-Acute Care Management provides enterprise-level leadership for post-acute strategy and Care Management services across Cedars-Sinai Medical Network. This role is accountable for the performance, quality outcomes, and strategic partnerships across the post-acute continuum—including Skilled Nursing Facilities, Home Health, Inpatient Rehabilitation, Long-Term Acute Care, and community-based transition partners.
The Senior Manager leads post-acute quality and utilization performance with a strong focus on readmissions reduction, network quality metrics, regulatory compliance, and value-based contract performance. Serving as the primary representative for post-acute partners, this leader builds and sustains high-performing relationships, drives corrective action when quality or utilization targets are not met, and aligns partners to system-wide population health and hospital throughput goals.
In collaboration with Medical Directors and executive leadership, the Senior Manager designs and implements policies, workflows, and programs grounded in current standards of care and regulatory requirements for post-acute services. This position provides strategic oversight of utilization management and referral processes, ensuring seamless transitions of care, appropriate level-of-care placement, and measurable improvements in patient outcomes and network performance.
Post-Acute Care Services
- Coordinates relationships with post-acute care providers—including skilled nursing facilities (SNFs), home health agencies, home infusion agencies, hospice, rehabilitation centers, long-term acute care hospitals (LTACHs), and other community-based partners—to ensure seamless transitions, high-quality outcomes, and alignment with organizational goals.
- Supports case management in their coordination and management of transitions of care across the continuum—not only from acute care to post-acute settings but also from ambulatory clinics and the emergency department (ED).
- Supervises provider performance, supports care management teams, and implements initiatives that reduce readmissions, improves patient satisfaction that optimizes both financial and clinical outcomes including supervising KPIs, length of stay, and post-acute utilization.
- Collaborates closely with frontline teams to support broader efforts to streamline and standardize the transitional care management (TCM) process holding partners accountable for care quality, efficiency and adherence to payer-neutral strategies.
- Facilitates regular meetings with providers to review data, share guidelines, and implement standardized protocols.
- Recommends and implements corrective actions as needed, in collaboration with both clinical and administrative leadership.
- Supports the build and implementation of annual monitoring plans and continuous improvement initiatives.
Primary Duties & Responsibilities
- Manages department labor, non-wage and capital budgets and identifies/justifies any variances that may occur
- Responsible for mentoring and training the management team who report directly to this role
- Collaborates with Physician dyad partners, Quality Management, Contracting, Network Management, Care Transition Programs and other departments to create an integrated, patient-centered care management program
- Responsible for supervision, evaluation, hiring, and counseling of all staff
- Establishes priorities to deliver clinical information solutions that are practical and effective
- Develops and monitors daily staff productivity standards and program evaluation outcome and in-process metrics; figures out priorities for process improvement, system optimization and training and recommends and implements tactics for performance improvements using PDCA methodology
- Uses Cedars-Sinai Daily Engagement (LEAN methodology) to develop and monitor leader standard work, empower front line staff and drive process improvement. Teaches and trains staff members on CSDE guidelines and encourages practice of the standards.
- Maintains excellent health plan relationships and ensures compliance with all health plan criteria
- Prepares and delivers provider and practice staff training.
- Contribute to the development and execution of service line strategies across the continuum of care.
- Collaborate with finance to ensure optimal cost management, partner alignment, and success under value-based care models.
- Identify and address service gaps or inefficiencies and recommend innovative solutions, including new partnerships, programs, or technologies.
Qualifications
- Bachelor's Degree required
- Master's Degree preferred
- 7 years of management experience in healthcare particularly managed care preferred
- 5 years Post Acute Care Management - experience in case management, care coordination, or post-acute care leadership. Must have deep understanding of post-acute care systems, transitional models, and population health strategies.
