Job Description
- The Lung Transplant Coordinator is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services—from initial referral to follow-up care.
- Serves as the primary coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff.
- Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community.
- Collects/documents data to meet clinical, financial, and regulatory requirements.
- Participates in the development of guidelines, protocols, pathways, and other performance improvement and research activities.
- Serves as a patient advocate throughout the patient care process.
Qualifications
- BSN degree required
- Current California RN License required
- BLS from the American Heart Association required
- CCRN Acute/Critical Care Nursing, PCCN Progressive Care Nursing or other specialty certification required
- Certified Clinical Transplant Nurse (CCTN) preferred (or willing to obtain within one year from start date)
- A minimum of 3 years of acute care nursing experience required
- At least 1 year of Critical Care experience required
- Organ transplantation experience preferred
- Demonstrated knowledge of case management required
- Experience in data management preferred
- Excellent organizational skills and ability to effectively coordinateall aspects of patient care required
