About the Cedars-Sinai Comprehensive Transplant Center

Position Overview
We are seeking a compassionate and dedicated Nurse Practitioner to join our team, providing exceptional care to our inpatient and outpatient Lung Transplant population. As a Nurse Practitioner in the Lung Transplant Program, you will play a vital role in the planning and coordination of patient care throughout the continuum of services—from initial referral to follow-up care.
Key Responsibilities
- Patient Coordination: Plan and coordinate all aspects of the work-up and care for Lung Transplant patients.
- Patient Evaluation: Conduct chart reviews, perform history and physical examinations, order diagnostic tests, review results, and report findings to the physician as needed.
- Inpatient Services: Provide inpatient services to Lung Transplant patients, coordinating daily activities including care coordination, patient education, and timely documentation of patient progress notes, discharge summaries, and CMS-required care coordination notes.
- Communication Management: Coordinate communication regarding patient admissions, discharges, transfers, and surgeries with referring and collaborating physicians.
- Patient Management Plans: Develop and implement patient management plans and assist in the provision of continuity of care.
- Education and Advocacy: Provide education, leadership, and patient care management across the healthcare continuum, from preventative and acute care through discharge. Serve as a patient advocate throughout the care process and represent Cedars-Sinai at regional and national meetings/conferences.
Qualifications
Education:
- MSN Degree required.
- Graduate of an accredited Nurse Practitioner program required (AGACNP strongly preferred).
License/Certification:
- Current CA RN License.
- Current Nurse Practitioner and Furnishing License.
- National Nurse Practitioner Certification (NPI).
- Current DEA License (schedule 2-5)
- Current ACLS certification.
Experience:
- A minimum of 3 years of acute care nursing experience preferred.
- Nurse Practitioner with relevant acute care experience preferred.
- Transplant and ICU experience strongly preferred.
